Job Position :KRCHN NURSES

USAID AMPATH Uzima is a Kenya Health Partnership for Quality Services (KHPQS) Program funded by USAID working in partnership with the Department of Health in Uasin Gishu, West Pokot, Trans Nzoia and Elgeyo Marakwet Counties.

The Academic Model Providing Access to Health Care (AMPATH) is a project under the auspices of Moi Teaching

and Referral Hospital (MTRH), Moi University and a Consortium of North American Universities led by Indiana University whose aim is to provide sustainable efforts in access to Health Care, Research and Training.

Applications are invited from suitably qualified candidates for the following vacant position in USAID AMPATH Uzima Program:

POSITION: KENYA REGISTERED COMMUNITY HEALTH NURSE (KRCHN)-(1 POSTS) SITE: UASINGISHU COUNTY

JOB GROUP: H

Job Summary:

Reporting to the CCC In-Charge, the successful candidate will among others be responsible for the following:-

Management of adherence and retention interventions for clients in HIV care.

Provision of nursing care to patients at the hospital and the Comprehensive Care Clinic with minimum

supervision.

• Supervise Peer educators, Mentor Mothers and lay counselors offering adherence counseling, Community • Conduct risk categorization of all clients and support provision of case management based on the clients needs

PHDP, and U-U messaging and services.

• Oversee the implementation of clients innovative models to improve early retention, reducing waiting time and

possibly under one roof with case management. • Conduct routine adherence counseling, and booster adherence counseling for patients with low-level viremia

and provide timely EACs for patients with high viremia.

⚫ Support case management and VDOTs for children with high viremia. Enhance the disclosure process in all populations with focus on children eligible for HIV disclosure with

caregivers. • Support OTZ and differentiated care models for children and adolescents in the facility.

Ensuring that all SOPs are followed at the facilities.

Ensure meetings for psychosocial support groups: pediatrics, adolescents, discordant, and PMTCT. • Ensure those clients on or being switched to second line are adequately prepared and appropriately documented. Ensure that all files have duly completed mandatory tools (adherence checklist, psychosocial baseline, family/

partner testing, ART treatment plan, completed locator form with sketch map, Pre ART-schedule, completed tracer forms for all clients that have been traced).

•Ensure proper management of clinic bookings and defaulter tracing, complete with documentation of outcomes.

• Attending support group meetings to give technical support for technical psychosocial issues to the educators.

Support in offering partner testing services to viremia clients and PMTCT mothers and liaising with HTS

providers to have elicited partners tested and documentation done.

Any other duties as may be assigned by your supervisor.

USAID AMPATH Uzima Qualification & Experience

Be a holder of Kenya Certificate of Education Division II or Kenya Certificate of Secondary Education Mean

Grade C (plain):

Be in possession of a Diploma in Community Health Nursing from a recognized institution; • Be registered with the Nursing Council of Kenya and should possess a valid practicing licence;

Have a minimum of two (2) years’ experience in a busy hospital: Have shown competence and ability in organizing performing and discharging duties and responsibilities;

Additional training in ART and PMTCT will be an added advantage;

Applicants should meet requirements of Chapter 6 of the Constitution of Kenya at the point of placement.

Terms of Employment

The successful candidate will be employed on 1-year renewable contract terms with competitive salary and

allowances with possible extension up to 30 September 2025 subject to availability of funds, satisfactory performance and need for service. of certificates.
